Skilled Nursing
Registered nurses (RN) and licensed vocational nurses (LVN) provide skilled services that cannot be performed safely and effectively by nonprofessional personnel. The RN will coordinate the patient's plan of care with physician(s), other health care professionals, and caregivers.

Assessment and education to patient and caregivers on acute and chronic diseases and treatments include but are not limited to:
New diagnosis or condition
Exacerbation of an existing diagnosis or condition
Bowel and bladder training
Gastrostomy feeding instruction
Foley catheter insertion and care

Physical Therapy (PT)
Physical therapists (PTS) work to restore the mobility and strength of patients who are limited or disabled by physical injuries through the use of exercise, massage, and other methods. They also teach patients and caregivers special techniques for walking and transfer. The goal of PT is to assist the patient in achieving their highest level of strength, endurance, energy, and function.

Occupational Therapy (OT)
Occupational Therapists (OTS) help individuals who have physical, developmental, social, or emotional problems that prevent them from performing the general activities of daily living. OTs instruct patients to improve their function in tasks such as eating, bathing, dressing, and basic household routines. The goal of OT is to increase physical and mental functionality, increase self-confidence, and assist the patient in achieving their highest level of independence in activities of daily living.

Speech Therapist (ST) helps individuals with communication, swallowing, breathing and cognition problems. The goal of ST is to improve and restore communication, swallowing, breathing and cognition.

Medical Social Work
Medical Social Worker (MSW) assist and counsel patients with health related financial, emotional, and social concerns as it relates to the patient's medical condition. These include but are not limited to:
Assessment of social and emotional factors

Identify and assist with appropriate community resources
Short and Long Term planning

Home Health Aide Services
Home Care Aides work under the direct supervision of a Registered Nurse or Therapist to assist patients who are limited in their ability to perform activities of daily living. Some of their duties may include:
Bathing, dressing and personal care
Catheter care and changing of catheter drainage bags
Empty or change colostomy bags
Personal grooming (hair, nail, and foot care)
Light housekeeping
Light meal preparation
Transfer and ambulation

Assist with non-personal care duties
